PDF - A Street Trench Excavation Permit is required for any excavation or occupation of a public way. It may also be required for subsurface excavation on private property.

Wetlands Permits may be required if you are planning any work within 100' of a wetland or vernal pool, or within 200' of a river or stream. PDF - Each business must file a personal property Form of List with the Assessors’ Office by March 1st. This form enables a business to provide a list of its current assets to the Board of Assessors.
PDF - MA State Tax Form 3ABC. Must be filed annually by charitable, benevolent, educational, literary, temperance, or scientific organizations and trusts owning real or personal property on January 1. Forms are due on March 1.

PDF - A printable mail-in request form for a certified copy of a marriage certificate. The record of marriage are available if the bride and groom filed for the marriage intention (application) in the town of Wellesley.
PDF - A printable mail-in request form for a certified copy of a birth certificate. Birth certificates are on file for births which occurred in Wellesley, or for children whose parent(s) were residing in Wellesley at the time of the birth.
PDF - A printable mail-in request form for a certified copy of a death certificate. Death certificates are on file for decedents who were pronounced in the Town of Wellesley, or who resided in the Town of Wellesley at the time of death.
