Dear Camp Joey, Camp Quest,Odyssey and Project Extreme Parents,
I would like to take this opportunity to welcome you to our 2012 summer camp program. Summer is quickly approaching and it is time to complete the paperwork so that your child may attend camp. These are Department of Public Health regulations and must be strictly followed. Failure to adhere to these regulations will mean that your child CANNOT attend camp. Here are the important due dates that you must be aware of:
a. Due ASAP
b. The H & E form is due May 15th for all weeks of camp. If we do not receive this form, your child will not be able to attend camp. Each Health & Emergency form must be filled out completely. You must also attach a copy of the child’s most recent physical exam and up to date immunization form, signed by their physician.
a. Due the Thursday before your child is to begin camp (by 4:30 pm)
b. The Authorization to Administer Medication to a Camper Form only needs to be completed if your child will require medications during camp hours. If they do then please fill out the form, sign it and return it to the Recreation Office by the Thursday before the camper is to begin camp. If you have any questions please contact the Recreation Department.
a. The Parent Packet includes basic information about camp as well as the pick up policy, sunscreen policy, and a special request form.
4. Field Trip Permission Forms
Please print & complete the field trip permission form for your child's session (s). Field Trip permission forms are due by the first day of your camp session.
Field Trips are not booked until May
Forms can be downloaded or can be picked up at the Recreation Office Monday – Friday 8:30 am – 4:30 pm.
